GLP-1 Drug and Food Interactions: What the FDA Labels Say
What can you eat, drink, and take with Wegovy, Ozempic, Zepbound, and Mounjaro? This guide pulls direct from the FDA labels and manufacturer patient guides. Not from forum posts or social media.

Alcohol
The FDA labels for Wegovy, Ozempic, Zepbound, and Mounjaro do not list alcohol as a direct drug interaction. But alcohol intensifies the most common side effects: nausea, vomiting, and reflux. It also empties calories at a time when intake is already low.
The bigger risk is hypoglycemia. If you take metformin, insulin, or a sulfonylurea alongside the GLP-1, alcohol can drop your blood sugar sharply. The Novo Nordisk patient guide and Eli Lilly patient guide both flag this and tell you to talk with your prescriber.
Many GLP-1 users report a sharp drop in desire to drink. Research at the University of North Carolina has begun studying GLP-1 medications for alcohol use disorder. Early signals are promising, but no GLP-1 is FDA-approved for that use yet.
Grapefruit
You can eat grapefruit on Wegovy, Ozempic, Zepbound, and Mounjaro. The current FDA labels list no clinically significant interaction. Semaglutide and tirzepatide are peptide drugs. They are not metabolized through the CYP3A4 liver enzyme that grapefruit blocks. This is one of the few times where the famous grapefruit warning does not apply.
If you take other medications alongside the GLP-1 (statins, blood pressure medication, anti-anxiety medication), grapefruit may still interact with those. Check each medication separately.
Oral Contraceptives (Birth Control Pills)
This is the most important interaction for women of reproductive age. The FDA label for tirzepatide (Zepbound and Mounjaro) carries a specific warning. Eli Lilly says oral contraceptives may be less effective because of delayed gastric emptying.
Eli Lilly recommends one of two options:
- 1. Switch to a non-oral birth control method (IUD, hormonal patch, ring, injection, implant)
- 2. Add a barrier method (condoms) for 4 weeks after starting tirzepatide and for 4 weeks after each dose increase
The FDA labels for Wegovy and Ozempic (semaglutide) do not carry this same warning. But the Novo Nordisk patient guide notes that delayed gastric emptying may affect absorption of any orally administered drug. Talk with your prescriber.
High-Fat Meals
Not a true drug interaction. But a real food-effect interaction that changes how the drug feels. GLP-1 medications slow gastric emptying. High-fat meals also slow gastric emptying. The two effects compound. Result: more nausea, more bloating, more fullness discomfort.
The Novo Nordisk patient guide recommends balanced meals with lean protein, vegetables, and modest fat. The Academy of Nutrition and Dietetics suggests 40 to 60 grams of fat per day for most GLP-1 users. Pick olive oil, nuts in small portions, avocado, and fatty fish over fried foods and rich sauces. Diabetes Medications
This is the most important drug-drug interaction. If you take insulin or a sulfonylurea (glipizide, glyburide, glimepiride) and you start a GLP-1, your risk of hypoglycemia goes up. The FDA labels for Ozempic and Mounjaro both flag this.
Your prescriber may lower your insulin or sulfonylurea dose when you start the GLP-1. Never adjust diabetes medication on your own. This needs prescriber guidance and blood sugar monitoring. Pick up a glucose meter or a CGM if you do not already have one.
Other Medications: Spacing Matters
Delayed gastric emptying can affect absorption of any oral medication. For most drugs, the effect is small. For narrow-therapeutic-index drugs, it can matter.
Drugs that may need spacing or monitoring:
- • Levothyroxine (thyroid medication) - take 30 to 60 minutes before food, check TSH levels
- • Warfarin (blood thinner) - check INR more often when starting GLP-1
- • Digoxin (heart medication) - check digoxin levels
- • Anti-seizure medications - discuss with neurologist
