What can you eat, drink, and take with Wegovy, Ozempic, Zepbound, and Mounjaro? This guide pulls direct from the FDA labels and manufacturer patient guides. Not from forum posts or social media.

Alcohol

The FDA labels for Wegovy, Ozempic, Zepbound, and Mounjaro do not list alcohol as a direct drug interaction. But alcohol intensifies the most common side effects: nausea, vomiting, and reflux. It also empties calories at a time when intake is already low.

The bigger risk is hypoglycemia. If you take metformin, insulin, or a sulfonylurea alongside the GLP-1, alcohol can drop your blood sugar sharply. The Novo Nordisk patient guide and Eli Lilly patient guide both flag this and tell you to talk with your prescriber.

Many GLP-1 users report a sharp drop in desire to drink. Research at the University of North Carolina has begun studying GLP-1 medications for alcohol use disorder. Early signals are promising, but no GLP-1 is FDA-approved for that use yet.

Grapefruit

You can eat grapefruit on Wegovy, Ozempic, Zepbound, and Mounjaro. The current FDA labels list no clinically significant interaction. Semaglutide and tirzepatide are peptide drugs. They are not metabolized through the CYP3A4 liver enzyme that grapefruit blocks. This is one of the few times where the famous grapefruit warning does not apply.

If you take other medications alongside the GLP-1 (statins, blood pressure medication, anti-anxiety medication), grapefruit may still interact with those. Check each medication separately.

Oral Contraceptives (Birth Control Pills)

This is the most important interaction for women of reproductive age. The FDA label for tirzepatide (Zepbound and Mounjaro) carries a specific warning. Eli Lilly says oral contraceptives may be less effective because of delayed gastric emptying.

Eli Lilly recommends one of two options:

The FDA labels for Wegovy and Ozempic (semaglutide) do not carry this same warning. But the Novo Nordisk patient guide notes that delayed gastric emptying may affect absorption of any orally administered drug. Talk with your prescriber.

High-Fat Meals

Not a true drug interaction. But a real food-effect interaction that changes how the drug feels. GLP-1 medications slow gastric emptying. High-fat meals also slow gastric emptying. The two effects compound. Result: more nausea, more bloating, more fullness discomfort.

The Novo Nordisk patient guide recommends balanced meals with lean protein, vegetables, and modest fat. The Academy of Nutrition and Dietetics suggests 40 to 60 grams of fat per day for most GLP-1 users. Pick olive oil, nuts in small portions, avocado, and fatty fish over fried foods and rich sauces. See our post-Wegovy diet plan for a sample meal layout.

Diabetes Medications

This is the most important drug-drug interaction. If you take insulin or a sulfonylurea (glipizide, glyburide, glimepiride) and you start a GLP-1, your risk of hypoglycemia goes up. The FDA labels for Ozempic and Mounjaro both flag this.

Your prescriber may lower your insulin or sulfonylurea dose when you start the GLP-1. Never adjust diabetes medication on your own. This needs prescriber guidance and blood sugar monitoring. Pick up a glucose meter or a CGM if you do not already have one.

Other Medications: Spacing Matters

Delayed gastric emptying can affect absorption of any oral medication. For most drugs, the effect is small. For narrow-therapeutic-index drugs, it can matter.

Frequently Asked Questions

Can I drink alcohol on Wegovy or Ozempic?

The FDA labels for Wegovy and Ozempic do not list alcohol as a direct interaction. But alcohol worsens GI side effects (nausea, vomiting, reflux). It also raises the risk of low blood sugar if you take metformin or insulin alongside the GLP-1. The Novo Nordisk patient guide says to talk with your prescriber about alcohol use. Many GLP-1 users report a sharp drop in desire to drink.

Does grapefruit interact with semaglutide or tirzepatide?

Per the current FDA labels for Wegovy, Ozempic, Zepbound, and Mounjaro, there is no clinically significant interaction with grapefruit. Semaglutide and tirzepatide are peptide drugs. They are not metabolized through the CYP3A4 liver pathway that grapefruit blocks. Grapefruit is safe with these medications.

Can GLP-1 drugs make my birth control pill less effective?

Possibly, for Zepbound and Mounjaro. The FDA label for tirzepatide (Zepbound/Mounjaro) warns that oral contraceptives may be less effective due to delayed gastric emptying. Eli Lilly recommends switching to a non-oral method (IUD, patch, ring, injection) or adding a barrier method for 4 weeks after starting tirzepatide and after each dose increase. The Wegovy and Ozempic labels from Novo Nordisk do not carry this same warning. Always check with your prescriber.

What foods slow how Wegovy or Ozempic works?

High-fat meals slow gastric emptying. GLP-1 drugs already slow it. The combination compounds the effect. This is why greasy, fried foods cause more nausea and fullness on GLP-1 medication. The Novo Nordisk patient guide recommends balanced meals with lean protein, vegetables, and modest fat to avoid this.

Can I take vitamins with Wegovy?

Most vitamins are fine. The FDA label for Wegovy does not flag direct interactions with standard multivitamins. But delayed gastric emptying can affect absorption of some drugs and supplements. If you take a critical medication (thyroid medication, anti-seizure medication, blood thinners), space it 30 to 60 minutes apart from your Wegovy injection day meals. Always confirm with your prescriber or pharmacist.

Does coffee interact with GLP-1 medications?

No clinically significant interaction is listed on the FDA labels for Wegovy, Ozempic, Zepbound, or Mounjaro. But coffee on an empty stomach can worsen nausea for some GLP-1 users. Eat a small protein snack first. Skip sugary coffee drinks (sweetened lattes, frappuccinos) because the sugar spike worsens GI side effects.

Can I drink protein shakes on GLP-1?

Yes. Protein shakes are one of the easiest ways to hit your protein target when appetite is low. Pick a whey isolate or plant-based shake with 20 to 30 grams of protein and low sugar (under 5 grams). Skip mass gainers and heavily sweetened shakes. The Academy of Nutrition and Dietetics endorses protein supplementation during rapid weight loss to preserve muscle.

Do GLP-1 drugs interact with diabetes medications?

Yes. If you take insulin or a sulfonylurea (glipizide, glyburide, glimepiride) alongside a GLP-1, your risk of low blood sugar goes up. The FDA labels for Ozempic and Mounjaro recommend that your prescriber may need to lower your insulin or sulfonylurea dose when starting GLP-1 treatment. Never adjust diabetes medication on your own. This requires prescriber guidance and blood sugar monitoring.

Can I take blood pressure or cholesterol meds with Wegovy?

Generally yes, but spacing matters. The FDA label for Wegovy notes that delayed gastric emptying may affect absorption of orally administered drugs. For most blood pressure and cholesterol medications, this is not clinically significant. For narrow-therapeutic-index drugs (digoxin, warfarin, levothyroxine), space them apart and have levels checked. Always run this past your prescriber.

Is caffeine safe on GLP-1 medication?

Yes. The FDA labels for Wegovy, Ozempic, Zepbound, and Mounjaro do not flag caffeine as an interaction. Stick to moderate amounts (under 400 mg per day, about 4 cups of coffee). Watch out for energy drinks and pre-workouts loaded with sugar and stimulants. These can spike blood sugar and worsen GI side effects.
